Background Check for Coaches / Administrators

All coaches, assistant coaches, and administrators must submit for and allow the Tennessee State Soccer Association to run a background check on behalf of the Gallatin Soccer Club. To begin that process visit the below link:

When visiting this link, you will be prompted with a page similar to the screenshot below. Click any screenshots to enlarge. At this point, do NOT login, but instead choose Registration in the upper right corner.

You will then be prompted with the below screen. Make sure that the proper season is selected and the “Coach / Admin Registration” is selected. At this point you can login if you have an existing Affinity account (from online signups perhaps) or you can create a new account.

On the next screen you will see the list of family members already associated with this account. If you already see yourself in the bottom section you can just click “Continue”. Otherwise add yourself as a Parent / Guardian.

You will be brought to this screen. For the appropriate name, click the button “Register as Coach/Admin”.

When prompted, choose “Background Checking” as the play level. Then you will need to verify that your information is correct including DOB, Drivers License, etc. You must answer all the Yes/No questions (offer explanations as necessary) and then click “Save & Next Page” when complete.

Please read through the information in the next screen. This is your consent for the Background Check, but also information related to Concussions and Sudden Cardiac Arrest. You must accept each of these and continue.

At this point you should be able to continue. The payment should be ZERO for the background check. You can think also print out a copy of the forms.
