For GSC coach use ONLY: ONLY current and actively registered coaches with GSC are permitted to submit this form.
Please carefully fill out the nomination form below.
First, provide your (the coach's) information. Second, nominate at least one player and answer the questions. You must obtain consent from the player’s parent or guardian for us to contact them regarding this new opportunity.
